Acknowledgment and Assumption of Risk:
I, the undersigned, hereby acknowledge and understand that participation in jiu-jitsu and related activities at Midgard Submission Grappling and Self-Defense (hereinafter referred to as the "Gym") involves inherent risks of physical injury, including but not limited to sprains, strains, dislocations, fractures, concussions, and other serious injuries or even death. I voluntarily assume all such risks associated with my participation in jiu-jitsu training and activities.
Waiver and Release:
In consideration of being allowed to participate in the activities and programs of the Gym, I hereby agree to release, waive, discharge, and hold harmless the Gym, its owners, agents, employees, instructors, trainers, and all other persons or entities acting in any capacity on its behalf (collectively referred to as the "Released Parties") from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation in these activities, including those allegedly attributed to the negligent acts or omissions of the Released Parties.
Medical Fitness and Consent to Treatment:
I hereby affirm that I am in good physical condition and do not suffer from any known condition which would prevent or limit my participation in jiu-jitsu activities. I agree to notify the Gym of any changes to my health or medical condition. In the event of an emergency, I consent to receive medical treatment, which may be deemed advisable in the event of injury, accident, and/or illness during my participation.
Indemnification:
I further agree to indemnify and hold harmless the Released Parties from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney's fees, brought as a result of my involvement in jiu-jitsu activities and to reimburse them for any such expenses incurred.
Photography and Media Release:
I grant the Gym the right to take photographs or videos of me during training and events and to use such media for promotional, educational, and marketing purposes, without compensation to me.
Miscellaneous Provisions:
Acknowledgment of Understanding:
I have read this Waiver and Release of Liability, fully understand its terms, and sign it freely and voluntarily without any inducement. I understand that I am giving up substantial rights by signing it.
Note: Participants under the age of 18 must have a parent or guardian read and sign this Waiver and Release of Liability on their behalf.
